Controlled Substances Policy

Pearl Cast Solutions is committed to safe, ethical, and evidence-based prescribing practices. The following policy outlines expectations regarding controlled substances.

General Prescribing Principles

Controlled substances are prescribed only when clinically appropriate and in accordance with federal and state regulations. Prescribing decisions are based on medical necessity, clinical evaluation, and ongoing monitoring.

Assessment and Monitoring

Patients prescribed controlled substances may be required to participate in periodic reassessments, urine drug screenings, prescription monitoring program (PMP) checks, and coordination with other providers.

No Early Refills or Replacements

Early refills, replacements for lost or stolen medications, or dose escalations without clinical justification are not provided.

Single Prescriber Policy

Controlled substances must be prescribed by a single provider unless otherwise coordinated and documented.

Medication Compliance

Failure to adhere to treatment recommendations, evidence of misuse, diversion, or inconsistent monitoring results may result in discontinuation of the controlled substance.

Right to Modify Treatment

Pearl Cast Solutions reserves the right to adjust, taper, or discontinue controlled substances at any time based on clinical judgment and patient safety.

Practice Boundaries

Pearl Cast Solutions is committed to providing ethical, professional, and patient-centered psychiatric care. The following practice boundaries are established to promote safety, clarity, and therapeutic integrity.

Professional Scope of Care

Services provided are limited to psychiatric evaluation, medication management, and clinically appropriate supportive interventions. PearlCast Solutions does not provide emergency services, intensive psychotherapy, or crisis intervention.

Appointments and Communication

Appointments are conducted by scheduled sessions only. Communication outside of scheduled visits is limited to clinical and administrative matters and must occur through approved channels. Response times may vary and are not immediate.

Emergency and Crisis Situations

Pearl Cast Solutions is not an emergency service. In the event of a psychiatric or medical emergency, clients should contact 911, go to the nearest emergency department, or call a local crisis hotline.

Boundaries and Dual Relationships

To maintain professional integrity, dual relationships outside of the therapeutic setting are not permitted. This includes social relationships, financial relationships, or personal engagements with clients.

Respectful Conduct

Mutual respect is expected at all times. Verbal abuse, threats, harassment, or inappropriate behavior may result in termination of services.

Continuity and Collaboration

Care may involve collaboration with other healthcare providers when clinically indicated and with appropriate consent.

Pearl Cast Solutions

At Pearl Cast Solutions, our policies are designed to support a structured, respectful, and effective therapeutic environment. Please review the following information carefully.

Appointments & Scheduling

Appointments are scheduled in advance and are reserved specifically for you. We recommend arriving on time and prepared for your session to ensure continuity and quality of care. A free 15-minute consultation is available to determine if services are an appropriate fit prior to scheduling a full evaluation.

Cancellation & No-Show Policy

If you need to cancel or reschedule an appointment, we require at least 24 hours’ notice. – Late cancellations (less than 24 hours’ notice) and missed appointments may be subject to a fee. – Repeated missed appointments may impact your ability to continue scheduling services. This policy helps ensure availability for all individuals seeking care.

Late Arrivals

If you arrive late to your appointment: – Your session may be shortened to accommodate scheduled appointments. – If significantly late, the session may need to be rescheduled and may be considered a missed appointment.

Fees & Payment

Payment policies will be discussed prior to the start of treatment. – Payment is due at the time of service unless otherwise arranged. A valid payment method may be required to remain on file. – Pearl Cast Solutions does not process payments directly through the public website; all billing is handled through a secure platform.

Insurance

Pearl Cast Solutions works with select insurance providers. – Coverage and benefits vary by plan. – Patients are responsible for understanding their insurance coverage, including co-pays, deductibles, and out-of-pocket costs.  Any services not covered by insurance will be the responsibility of the patient. Please contact the office for the most current list of accepted insurance plans. 

Telehealth Policy

Telehealth services are offered in accordance with New York State regulations. By participating in telehealth services, you agree to:

– Be physically located in a state where the provider is licensed at the time of the session
– Be in a private, secure, and quiet environment
– Avoid attending sessions while driving or in public spaces

Telehealth is not appropriate for emergencies or crisis situations.

In-Person & Community-Based Services

When applicable, in-person services may be provided in community settings. Patients are expected to:

– Be available at the agreed-upon location and time
– Maintain a safe and appropriate environment for the session

Medication Management

Medication management is provided when clinically indicated.

– Prescriptions are issued based on clinical assessment and medical necessity
– Patients are expected to take medications as prescribed and report side effects or concerns
– Refills should be requested in advance; last-minute refill requests may not be accommodated
-Certain medications may require closer monitoring, follow-up appointments, or additional documentation.

Communication Policy

Communication outside of scheduled sessions is limited to administrative or clinical needs. – Non-urgent matters may be addressed via secure messaging or phone – Response times may vary and are typically within standard business hours – This practice does not provide 24/7 on-call services.

Emergency & Crisis Policy

Pearl Cast Solutions is not an emergency service.

If you are experiencing a mental health emergency:

– Call 911 – Go to your nearest emergency room
– Contact the 988 Suicide & Crisis Lifeline

For safety reasons, urgent or crisis-related concerns should not be communicated via email or contact forms.

Confidentiality & Privacy

All services are provided in accordance with HIPAA regulations to protect your privacy.

Information may only be disclosed:

– With your written consent  
– As required by law (e.g., risk of harm to self or others, abuse reporting)

Treatment Participation

Effective care requires active participation. Patients are encouraged to:

– Engage honestly in sessions  
– Participate in treatment planning  
– Follow agreed-upon recommendations  
-Treatment outcomes are best achieved through a collaborative and consistent approach

Right to Modify Policies

Pearl Cast Solutions reserves the right to update or modify these policies at any time. Updated policies will be reflected on the website.
